Technical/Scientific Update

This session will review the latest research findings for infant feeding in the context of HIV and describe the best infant feeding options for HIV positive women. The presenter will also review the importance of maternal health in infant feeding, risks of replacement feeding and early weaning. This session will provide the scientific justification for the proposed ICAP approach to infant feeding and enhanced HIV survival.

What is the ICAP Approach?

This session will also propose an ICAP approach to be adopted and adapted by country programs. Considering the latest findings on infant feeding and HIV the moderator will guide a session to elicit consensus and discussion about the new ICAP approach to infant feeding. The outcome of this session will be part of the ICAP Approach to Infant Feeding.

Country Team Strategizing: Optimizing the Approach to Infant Feeding and Action Oriented Dissemination to the Local Level

The country team will review and agree on the variety of means and action steps available to implement the discussed session/program element. The team will begin to draft a workplan document that shows activities and results as well as the conditions necessary for achieving and monitoring both. They will define how activities will be monitored, progress evaluated and data collected and used to support program implementation. These conditions include important assumptions on which rest decisions about activities, location, timing, procurement, and so on.

Tygerberg Experience & ICAP Approach
Presenters: Justin Engelbrecht, Nocawe Frans, Helena Rabie
This session will describe and outline the key elements of a comprehensive pediatric adherence program including preparation activities, counseling and supportive services, adherence monitoring, and appointment/ patient tracking systems. Presenters will also describe experience with and practical approaches to giving medication to infants and children. Will include an optional drug taste test.
Tygerberg Experience: Developmentally Appropriate Disclosure Counseling and Support
This session will describe Tygerberg’s experience with addressing the complex issues around disclosure of HIV status to an HIV infected child. This includes outlining the pros and cons of disclosure, framing disclosure as an ongoing process, describing a developmental appropriate approach to disclosure, outlining family responses and expectations around disclosure, listing obstacles to disclosure and sharing Tygerberg approaches to answering a child’s questions about the illness in an age appropriate manner.

Adherence and Disclosure from a Caregiver’s Perspective:

This session will be presented by caregiver(s) to discuss adherence issues from the family perspective. Caregivers will describe challenges of caring for the HIV infected child, adherence issues as well as challenges they face communicating with providers and engaging the health care service system and other complementary resources.
3:30-4:30 p.m.
ICAP Approach to Pediatric Adherence and Disclosure
Presenters: Peter Twyman and Tayla Colton
Participants will be divided into 8 small discussion cross-country groups and will be asked to identify strategies for addressing key elements of pediatric adherence and disclosure. Small groups will be assigned one of the following four program areas: 1. Facility based adherence and disclosure counseling for caregivers with infants, 2. Facility based adherence and disclosure counseling for older children, 3. Patient tracking systems and 4. Community linkages and support related to adherence and disclosure. Each group will be asked to identify the following: strategies for their respective program area, tools and resources needed to support the strategies, responsible persons, and anticipated challenges. Groups will be asked to distinguish between adherence to care and adherence to treatment. Each group will briefly report back the highlights of their discussion.
